Where is your clinic?
The Immunization Clinic is located at 4813 New Haven Avenue, at the corner of New Haven and Meyer Road. Parking is free and the clinic is located along a Citilink bus route.

What are the clinic hours?
Clinic hours are 8:30-11:30 a.m. and 1-4 p.m. Wednesday and Friday.

Do I need an appointment?
Yes, we are now requiring appointments. Please call (260) 449-7514 to make an appointment.

What do I need to bring for my child to receive immunizations?
Bring the child's current/up-to-date record.

Is there a cost for the shots?
There is no charge for immunizations for infants and children through 18 years of age. There is a charge for some adult vaccines. Donations are very helpful and much appreciated.

Who can bring my children for their shots?
A parent or legal guardian may bring a child in for shots. Other friends or relatives must have notarized permission (must not be expired) from the parent.

How many shots will my child receive?
We review the child's shot records and recommend the vaccines that are needed at the time. We administer all the vaccines a child needs at the time of the visit.

How do I obtain a shot record?

If your child has been to the Fort Wayne-Allen County Department of Health's Immunization clinic, you can come to the clinic during normal hours to request a shot record. We do not give out the dates of shots over the phone. You may request that we fax a copy of the record to your physician's office.

Are you a travel clinic?
We are not a travel clinic. We can provide information about travel vaccines and where to get them, but we do not provide vaccinations for travel.

Do you provide lice shampoo?
No. We do not provide lice shampoo.
