General Information

The mission of the HIV/STD Prevention Program is to intervene in the spread of HIV and other sexually transmitted diseases (STDs) and to reduce the complications of these diseases.  Prevention activities include contact tracing, screening, and providing education in a timely manner.  Intervention systems include:  surveillance, case detection through screening, treatment of known cases, case follow-up, and education.

STD Appointments:    

Calls are taken starting at 8:00 am to schedule an appointment for the following day. Due to the high demand for appointments, it is best to call (260) 449-7504 as early as possible for your best chance to get an appointment.  

Walk-in appointments are available. Clients are seen on a first-come, first-served basis; it is suggested that you arrive at the time posted in parenthesis for the best chance to be seen.   

  • Monday PM (1:00 p.m.)  
  • Wednesday PM (1:00 p.m.)  
  • Thursday AM (8:00 a.m.)  
  • Friday PM (1:00 p.m.)

For all scheduled and walk-in appointments, you are encouraged to print and complete the "Intake Form" found at the link below, and bring to the clinic for your appointment.
