General Information
The Vital Records division is responsible for collecting, preserving and certifying records of vital events in Allen County. Records available from the Health Department include records of births and death as well as affidavits of paternity. These records are collected from hospitals, funeral directors and the courts. Each record received is inspected for completeness, accuracy and legibility, arranged in date order, assigned a local number and processed.
Certified copies of birth and death certificates for events occuring in Fort Wayne and Allen County may be obtained through walk-in service or by mail. The cost is $10 for a birth certificate and $12 for a death certificate.

For requests received through the mail, please allow a 7 to 10 days for processing. When mailing a request you must send a money order made payable to Vital Records. We are not responsible for cash sent as payment.
