The Vital Records division is responsible for collecting, preserving and certifying records of vital events in Allen County. Records available from the Health Department include records of birth and death as well as affiidavits of paternity. These records are collected from hospitals, funeral directors and the courts. Each record received is inspected for completeness, accuracy and legibility, arranged in date order, assigned a local number and processed.
Certified copies of birth and death certificates for events occuring in Fort Wayne and Allen County may be obtained through our walk-in service, mail-in service, or using an online service: VitalChek.
Effective April 1, 2013: the cost of certificates using our walk-in or mail-in service is $11 for a birth certificate and $13 for a death certificate. We accept cash, money order, debit or credit cards (there is a minimum processing fee of $1.50 per credit card transaction). We do not accept personal checks. We are not responsible for cash sent through the mail as payment. For requests received through the mail, please allow 7 to 10 days for processing after we receive your request.

Death Records begin in 1871 for persons who died inside the city limits of Fort Wayne and 1907 for persons who died outside the city limits.
