Visit the Vital Records Office on the third floor of the Citizens Square Building, 200 E. Berry St., Fort Wayne, IN 46802. The Vital Records Office is open Monday-Friday, 8:00 AM. to 4:30 P.M. You can park in the parking lot off Berry Street or there is metered parking nearby.

Applicants for Vital Records must have a direct and tangible interest in the record requested and must be able to identify it with reasonable particularity. Identification is required for the purchase of a Birth or Death certificate. Options include a valid driver's license, state ID or passport. If you do not have any of these documents, we can accept two documents with your signature. Please, click here to view the list of acceptable forms of identification. If you are seeking a birth certificate and are not a person named on the document, you will need to contact our office at (260) 449-7147 for further instructions.
After completing a form, your average wait time to receive your birth or death certificate is 10 to 20 minutes.

Death Records begin in 1871 for persons who died inside the city limits of Fort Wayne and 1907 for persons who died outside the city limits.
