Property Records Assessment and/or Phase I Requests involve a records review of real estate and the surrounding area to determine any potential or existing environmental hazards or contamination.
To request a copy of a Property Records Assessment, please download or print a copy of the Property Records Assessment and Request Form. Completed forms can be returned to info@allencountyhealth.com; faxed to 260.427.1391; or mailed to or dropped off at 200 E. Berry St., Suite 360 Fort Wayne, IN 46802.
Requests are completed in the order in which they are received. We attempt to fulfill requests within 7 to 10 business days.
