A Property Records Assessment Requests and/or Phase I Requests involves a records review of a real estate property and the surrounding area to determine any potential or existing environmental hazards or contamination, usually from prior toxic chemical use or disposal.
On December 14, 2018, the Allen County Commissioners approved amendments to Allen County Code Title 10 (Department of Health) – Article 1 (Miscellaneous Fees). One of the amendments was the addition of a $25 fee to be charged to those requesting the creation and provision of the Phase I Environmental Summary Report from the Allen County Department of Health regarding documents on record with the Department for various properties in Allen County. These amendments took effect January 1, 2019.
The Allen County Department of Health will continue providing the information that has been provided for years, but you will now have two options for obtaining the information. The change was precipitated because Property Record Records Assessment Requests and/or Phase 1 requests have increased significantly – requiring a great deal of staff time. The reports take approximately 1-2 hours to compile and complete, and this is more of a service being provided than just a simply access to records. Therefore, we wanted to offer a choice to meet our client’s needs, and be able to continue providing the summary report if requested, but with this nominal fee to cover our costs in providing this valuable service. There are 2 options in how the records can be obtained:
- You can visit the Allen County Department of Health office to view the records yourself and request copies. There will be no charge for the records search, except 10 cents per copied page for 10 or more pages (per Allen County ordinance for copies of public records). The visit will need to be scheduled to ensure the necessary files are pulled and an employee is available to assist. - You can request the Allen County Department of Health to craft a formal summary report of the records you are requesting and the Allen County Department of Health will mail/email them to you. There will be a $25 fee for this request. Payment must be submitted via cash or check paid to the Allen County Department of Health before the report will be provided. The payment and request form can be mailed to the Allen County Department of Health, 200 E Berry St, Suite 360, Fort Wayne, IN 46802 or dropped off at the same address. Payment hours are Monday-Friday, 8 a.m.-4 p.m.
Requests are completed in the order in which they are received. We attempt to fulfill requests within 7 to 10 business days.
